Painful Menses: Leading Indications In Dysmenorrhea

Aconite:- Congestive type with violent headache, flashed
face; labor like pressing in womb; necessity to bend double on
account of pain, but finds relief in no position; restless; sup-
pressed menses from fright.
Alumina:- Menses too early, too short and too scanty (Nux-
vom); blood pale; pain in abdomen before and during menses.
After menses she is so exhausted in mind and body that the
slightest effort prostrates (Cocculus). Acrid, excoriating, trans-
parent mucous discharge before and after menses.
Ammonium Carb:- Menses too soon; abundant; (may be
scanty); blood clotted, black and passed with colicky pains; hard
difficult stools; pale face, toothache, backache, chilliness and
pain in abdomen during menses. Itching, burning and swelling
of pudendum (Senecio Grac.)
Apis Mel:- Inflammation, induration and swelling of right
ovary (Bell) with sharp, cutting and stinging pains during
menses. Pressing down pain in uterus. Scanty dark urine
(Lyc., Bell, Nit. Ac.) Wax colored skin.
Baryta:- Discharge of bloody mucus. Very weak; has
fainting spells (Moschus). Menses scanty. Troublesome
weight above pubes. Glandular enlargements.
Belladonna:- Strong bearing down as if everything would
protrude through vulva (Sepia, Platinum). Pains come suddenly
sight. Redness and bloatedness of face, dilated pupils, throbbing
red and offensive. Violent pain in small of back and bearing
down. Rage, wants to bite and to escape. Congestion of uterus
and labia (Atropine).
Berberis Vulg:- Catamenia too scanty, painful (in small of
back). Menses consisting of gray mucus or brown blood.
Bufo:- Menses too early with headache.
Bromium:- Some hours after menstrual flow has commenced,
Loud emissions of flatulence from vagina (Lyc.) Hard swelling
lying down. Terrible pains which make her cry out and weep
(Puls. Cham. Cuprum. Nux.) Periodical pains in evening.
Constricted pain in region of heart as if grasped by iron hand.
Palpitation (Lillium) when lying on left side.
Calcarea Carb:- Various complaints accompanying any
monthly flow. Swelling and tenderness of breasts, headache,
colic, shivering and leucorrhoea precede flow, cutting pain in
abdomen and bearing down in genitals during flow and toothache
following. Feet cold as if covered with damp stockings. Catamenia
Calcarea Phos:- Dysmenorrhoea resulting from carelessness
Acid Nit:- Painful abdominal spasms during menses.
Discharge scanty and brown. Soreness of mammae at each
period (Calc. Carb.) Urine stops and starts to flow several
times during urination.
Gelsemium:- Dysmenorrhoea preceded by sick headache,
Congestion to head, red face (Acon. Bell.) and bearing down in
abdomen. Pain in back and limbs (Cimicifuga). Loss of voice
during menses. Weakness.
Ignatia:- Menses too early and too profuse, or scanty,
black, clotted and putrid (Puls) Uterine spasms with crampy
pressing during menses by pressure in recumbent position. All
gone sensation at stomach. Tearful.
Caulophyllum:- Spasmodic bearing down pain with scanty
flow. Sympathetic cramps in rectum and bladder. Hysterical
spasms of chest and larynx. Membranous dysmenorrhoea (Ci-
micifuga, Collin.) Headache with heat and heaviness in head.
Chamomilla:- Drawing pains from back to front. Blood
dark and coagulated with tearing in thighs (Cimi.) Pains,
labor-like (Acon.) Bloated face or one cheek pale and the
other red. Hot perspiration about head. Very impatient,
can't answer one civilly, can't bear the pains.
Cimicifuga:- Severe pain in back, occiput and vertex.
Pains through hips and down thighs of a neuralgic character.
Labor-like pain with heavy pressing downward (Cham.) Hysterical
Cocculus:- Dysmenorrhoea always followed by hemorrhoids.
Irregular labor-like pains, one severe one followed by several
light ones. Tired by the least conversation. Nausea and vomiting
Coffea:- Great nervous erythism.
Lachesis.:- Pains relieved by flow. Menstrual colic beginning
Magnesia Carb:- Menses preceded by sore throat, which
continues until end of flow and is worse at night. Blood thick,
dark and pitch-like, difficult to remove from linen. During
pain there is no flow. Flow at night. During menses weakness,
(Gels.) chilliness, headache, pale face, and pain in small of back.
Lillium Tig:- Pain extends across hypogastrium to groin
and down leg. Menses dark, thick, and smell like lochia
(Kreosot.) Severe neuralgic pain in uterus. Slightest jar aggravates.

Pulsatilla:- Delayed menses. Discharge thick and dark,
flowing by fits and starts (Kreosote). Pains violent, causing
anguished tossing about with weeping (Cimicif.) Chilliness
and paleness of face. All symptoms worse in warm room. Mild
and tearful disposition.
Sepia:- Preceding menses, excoriating leucorrhoea. Menses
gone sensation at pit of stomach (Ignatia.) Colicky pains and
bearing down. Sallow complexion, yellow saddle across the
bridge of the nose.
Sulphur:- Blood thick, black and acrid, corroding the
thighs. Coustant heat on top of head (Cimicif.) Frequent
flashes of heat (Nitrate of Amyle.) Weak, fainting spells
(Baryta, Moschus.) Spotted red face. Pains by standing.
Chronic eruptions here and there. Suitable to thin people who
walk stooping.
Veratrum Alb:- Before menses, headache, vertigo, nosebleed,
and night sweat. During menses, morning headache, nausea,
ringing in ears and pains in all limbs. After menses, grinding
of teeth and bluish face. Hands and feet ache with the cold.
is suddenly seized with terrible colicky pains in womb and lower
part of abdomen, which continues sometimes for hours. Cramps
in legs. This remedy is similar to cimicifuga and caulophyllum.
Kreosotum:- Hardness of hearing before and during menses,
Natrum Mur:- Melancholy, and colic during menses.
Headache preceding, during and after menstruation. Pressing
and bearing down in genitals. Craves salty food.

Homeopathic Medicines for Dysfunctional Uterine Bleeding

Some commonly indicated homeopathic medicines for Dysfunctional Uterine Bleeding are following.
1. Sepia:- Bearing down sensation as if everything would escape through vulva;must cross limbs to prevent protrusion or press against vulva.
Menses too late and scanty, irregular; exceptionally menses may be early and profuse; sharp clutching pains. Violent stitches upward in the vagina from uterus to umbilicus. Vagina painful, especially on coition.
Hot Flashes at menopause with weakness and perspiration.
Sepia patient dreads to alone, weeps when telling her complaint, irritable, very sad and Indifferent to those loved most.
2. Lachesis :- Lachesis has hemorrhagic tendency. Very important during the climacteric. Cannot bear anything tight anywhere.
Climacteric troubles, palpitation, flashes of heat, hemorrhages, vertex headache, fainting spells worse pressure of clothes.
Menses too short, too feeble; pains all relieved by the flow. Left ovary very painful and swollen, indurated.
Great loquacity.Jealous.
3. Phosphorus:- Hemorrhage from uterus between periods. Menses too early and scanty- not profuse but last too long.Weeps before menses.
Frequent and profuse hemorrhages from uterus caused by cancerous affection.
Weakness, blue circles around eyes and anxiety after menses.
Infertility.
4. Calcarea Carbonica:- Menses too early, too profuse, too long with vertigo, toothache and cold,damp feet. Least excitement causes hemorrhage from uterus.Cutting pains in uterus during menstruation.
Before menses, headache, colic, chilliness and leucorrhea.
Infertility with copious menses.
Increased menstruation about external genitals.
5. Pulsatilla:- Menses too late, scanty, thick, dark, clotted, changeable, intermittent.Menses suppressed from wet feet, nervous debility or anemia.
Chilliness, nausea, downward pressure, painful with intermittent menstrual flow.
Weeps when telling complaints, changeable, contradictory. Patient seeks the open air and always feel better there. Symptoms ever changing. Thirstless, peevish and chilly.
6. Ammonium Carb:- Menses too frequent, profuse, early, copious, clotted, black.Menses preceded by gripping colic, black and acrid blood.
Cholera like symptoms at the commencement of menstruation.
7.Lycopodium:- Menses too late, last too long, too profuse. Coition painful, dry vagina. Right ovarian pain. Discharge of blood from genitals during stool.
Melancholy; afraid to be alone.Little things annoy. Headstrong and haughty when sick.
8.Ambra Grisea:- Thin, scrawny women. Adapted to hysterical subjects. Anemic and sleepless. Weakness,coldness and numbness,usually of single parts, fingers, arms etc. Music aggravates symptoms.
Menses too early. Discharge of blood between periods, bleeding at every little accident. Profuse,bluish leucorrhea.
Itching of pudendum with soreness and swelling. Worse at night.
9.Arsenicum Album:- Arsenic is useful in persistent hemorrhages i.e. metrorrhagia or menorrhagia, of a low type depending upon some degeneration in the organ affected. Burning pains with hemorrhage.
Metrorrhagia of dark blood and increased sexual desire. Main affection in right ovary with marked burning, tensive pains and restlessness which is somewhat relieved by constantly moving the feet. Menstrual colic better from warm applications.
10. Belladona:- Menses are early,copious, bright red and attended with cramp-like tearing pain in the back, arms etc; throbbing headache; most intensely painful congestive dysmenorrhea; bearing down; cutting pain from behind forward or vice versa; menstrual flow offensive; lochia offensive. Uterine hemorrhage; blood pours out and feels hot; uterine hemorrhage with bearing down in the back.
11. Bovista:- Menses too early and profuse; worse at night. Diarrhea before and during menses. Cannot wear tight clothing around the waist. Traces of blood between menses. Soreness of pubis during menses. Metrorrhagia.
Leucorrhea acrid, thick, tough, greenish follows menses.
Uterine hemorrhage when the uterus is engorged, particularly when there is bleeding between menses from any little over-exertion. Here it is exactly like Ambra Grisea, but menstrual flow of Bovista occurs chiefly or only at night or early in the morning.
12. Cantharides:- Puerperal metritis with inflammation of bladder. Menses too early and too profuse; black swelling of vulva with irritation.
Constant discharge from uterus; worse false step. Burning pain in ovaries.
Retained placenta with painful urination.
13. Carbo Animalis:- Menses too early, frequent, long lasting, followed by great exhaustion, so weak can hardly speak. Menstrual flow only in morning. Burning in vagina and labia.
Cancer of uterus.
14. carbo Veg.:- Premature, too copious menses, pale blood. Continuous dark hemorrhage. Patient wants to be fanned, anemic, skin cool and bluish, pulse rapid and weak.
Burning pains across the sacrum and lower portion of the spine with hemorrhage.Carbo veg is torpid, sluggish.
15. Cinchona Officinalis:- Menses too early, profuse with pain. Leucorrhea bloody. Bleeding is dark and clotted. Hemorrhage is so profuse it produce condition of collapse, cool face, gasping for breath, patient demands to be fanned.
16. Erigeron Canadensis:- Hemorrhage from uterus with painful micturition. Profuse bright red blood. Pain in left ovary and hip.
Metrorrhagia with violent irritation of rectum and bladder; and prolapse of uterus. Menorrhagia. Bloody lochia returns after least motion, comes in gushes; between periods, leucorrhea with urinary irritation; pregnant women with “weak uterus”, a bloody discharge on slight exertion.
Erigeron is indicated in hemorrhages from uterus with painful urination. Hemorrhage comes in fits and starts; it comes with a sudden gush and then stops again.
17. Ferrum Metallicum:- Anemic women with fiery red face whose menses remit a day or two and then return.
Menses too early, too profuse, last too long; pale , watery. Sensitive vagina.
Discharge of long pieces from uterus.
Tendency to abortion. Prolapse of vagina.
Flow of bright red blood often mixed with coagula, associated with great deal of flushing.
18. Hamamelis Viriginica:- Menses dark, profuse, with soreness in abdomen. Bruised soreness of affected parts.Metrorrhagia occurring midway between menstrual periods. Inter menstrual pain.Vicarious menstruation.
Ovarian congestion with neuralgia; feel very sore.Uterine hemorrhage, bearing down pain in back.
Vagina very tender. Profuse leucorrhea. Vaginismus, ovaritis, soreness over whole abdomen.
The flow is dark and rather passive and there is a feeling of soreness in the affected area.
19.Ipecacuanha:- Hemorrhage bright red and profuse.
Uterine hemorrhage profuse, bright red, gushing with nausea. Pain from naval to uterus.
Menses too early and too profuse.
20. Millefolium:- Invaluable remedy for various types of hemorrhages; blood bright red.
Menses early, profuse, protracted. Hemorrhage bright red, fluid.
Millefolium is suited for a profuse, bright red flow , unattended with pain.
21.Nux Vomica:- Menses too early, lasts too long; always irregular, blood black with faint spells.
Prolpase of uterus. Dysmenorrea with pain in sacrum and constant urging to stool.
Metrorrhagia with sensation as if bowels wanted to move.
22. Sabina:- Menses bright red, clotted and worse from any motion. Hemorrhage is attended with pain extending from the pubes through to the sacrum and with pains in the legs. Uterine pains extend to thighs.
Discharge of blood between periods with sexual excitement.
Retained placenta; intense after pains.
Menorrhagia in women who aborted readily. Inflammation of ovaries and uterus after abortion.
Pain from sacrum to pubis and from below upwards shooting up in the vagina.
23. Sanguinaria Can:- Metrorrhagia occurring at the climacteric age. Bleeding is of bright red, clotted and frequently offensive. Metrorrhagia with flushes of heat and with sick headache.
24.Secale Cornutum:- Hemorrhages; continued oozing; thin, fetid, watery black blood. Whole body is pervaded by a sense of great heat so all the Secale patients are better from cold.
Menses irregular, copious, dark; continuous oozing of watery blood until next period.
The flow of blood is passive; it is attended with tingling in the limbs.
Although the surface of the body is cold, the patient persistently expresses her desire to be uncovered.
25. Trillium Pendulum:- A general hemorrhagic medicine with great faintness and dizziness.
Uterine hemorrhage with sensation as though hips and back were falling to pieces; better by tight bandages.
Gushing of bright blood on least movement.
Metrorrhagia at climacteric. Hemorrhage from fibroids.
Prolapse with great bearing down.
Blood is bright red, profuse and is attended by a faint feeling in the epigastrium, pain in back, coldness of extremities, prostration and rapid, feeble pulse.
26. Ustilago Maydis:- Vicarious menstruation. Profuse menstruation after miscarriage; discharge of blood from slightest provocation; bright red; partly clotted.
Menorrhagia at climacteric.
Oozing of dark blood, clotted, forming long black strings. Cervix bleed easily.
Ovaries burn, painful and swelled.
Uterine hemorrhage is partly fluid and partly clotted, bright red.

Pelvic Inflammatory Disease( PID)
Pelvic Inflammatory disease is a disease of young women who are sexually active and in child born age. Pelvic inflammatory disease is Inflammation of woman's upper genital tract or reproductive organs (cervix, uterus, fallopian tubes,ovaries and surrounding tissues), main victims are fallopian tubes and ovaries. Bacterial infection is most common cause of PID.

How Nature Protect Reproductive Organs:
There are many natural barriers which prevent entry of bacteria from cervix to upward.
1.In Virgin girls intact Hymen prevents ascending infections.
2.Acidity of Vagina inhibits the bacterial growth.
3. Cervical canal has relatively small lumen and it is filled with a plug of alkaline mucus.
4.There are fimbrias in uterus and cervical canal which constantly move downwards
and so they discourage the spread of non-motile organisms further upward.
But this natural protective mechanism breaks during menstruation(periods), after abortion and delivery. During these events cervical canals dilates, fimbrial layer of uterus and cervical canal damages . Vaginal pH increases ( means vagina become more basic) and give favorable environment to germinal growth.
Manipulation like curettage for evacuation in abortion and manual removal of placenta give chance to entry of bacteria.
Intrauterine contraceptive device (inserting Copper T and other devices) may also be a source of infection.
Causes of PID
A- Multiple sex partner and frequent change of sex partner
B- Anemia- Which reduces the immune capacity to fight infections
C- In developing and poor countries most of the deliveries are conducted at home by untrained personals in aseptic environment. Criminal abortions are continue.
Infections Which causes PID
More than 40 type of organisms causes PID but most common infections are Chlamydia and Gonorrhea, both these organisms are transmitted sexually . More than 50% cases are infected by these 2 type of organisms.
What Happen in PID
Bacteria infects the fallopian tubes and cause inflammation. When this happens, normal tissue can become scarred and block the normal passage of an egg(block fallopian tube-either partial or full blockage). Fallopian tubes may damage.You may become infertile (unable to become pregnant). But if your tubes are partially blocked, an egg may implant outside the uterus and cause a dangerous condition called an ectopic pregnancy.In this way the worst outcome of PID could be Infertility in women.
PID has two main complications
1. Infertility (unable to become pregnant)
2. Ectopic Pregnancy( Implantation of Fetus outside the uterus) it may be a dangerous condition.
An ectopic pregnancy can cause internal bleeding leading to death.
Outlook of PID
1. According to US studies --Following a single episode of PID, 8% of women were infertile; after 2 episodes, 19.5% of women were infertile; and after 3 or more episodes, 40% of women were infertile.Tubal damage and scarring can result in infertility. PID is the most common cause of infertility in women.
2. Ectopic pregnancy is 6 times more common in women who have had PID at least once.
3.Chronic pelvic pain is present in up to 18% of women with PID.
4.Ovarian abscesses can occur after an episode of PID.Chronic infection may cause TOA. A TOA(tubo-ovarian mass) is a collection of bacteria, pus, and fluid that occurs in the fallopian tube. It is most often seen in teens. A TOA is also more likely to occur in teens or adult women who use intrauterine devices (IUDs) as birth control.
5.In Developing countries like India Criminal abortions are common. Many deliveries are conducted at home by untrained professionals which causes Pelvic Inflammatory Disease later on. It is estimated that 50 % pf al PID cases of developing countries are caused in this manner.
Symptoms of PID
1.Pain- Lower abdominal pain and Tenderness( pain on touch) which get worse before menstruation.
2. Back Pain
3. Heavy/foul smelling Vaginal Discharge
4.Abnormal Uterine Bleeding( Metrorrhagia)
5.Painful Sexual Intercourse (Dyspareunia)
6.Painful Urination
In Acute Condition
1.Fever
2. Nausea and Vomiting may coexist with PID.
Laboratory Tests
1. Complete Blood count.
2. Complete Urine test
3. Culture for gonorrhea and chlamydia
4. Test for other STD's and HIV.
Imaging
1.Pelvic Ultrasound
2.Laproscopy if indicated.
Treatment of PID
1. Bacterial samples are hard to collect from upper genital tract. So two type of Wide range Antibiotics can be used either orally or intravenously as indicated on intensity of symptoms.
2. Hospitalization may be necessary in severe cases or if PID occur during pregnancy(Although it is very rare).
3. Stop sexual activity until treatment is not completed. Sexual partner should also be treated to stop recurrence.
Complications Of PID
1.Infertility(unable to pregnant)
2.Ectopic pregnancy( Fertilized egg means embryo can implant outside the uterus it may be dangerous.)
3.Chronic PID -if acute infections not treated properly then chronic PID can develop.
Prevention Of PID
Follow few steps to remain healthy:-
1. Wipe from front to back after stool to prevent bacterial entery to vagina.
2.In case of vaginal itching,do not scratch ,just wash with water.
3.Frequent Vaginal douching may push bacteria to inside. So try to escape from frequent use of Vaginal douching.
4. Multiple sex partner is a potential risk of PID. Use barrier devices as latex condoms. Intrauterine Device (copper-T) does not prevent from Sexually transmitted diseases. Limit your sex partners and specially high risk partners.
